Just about all doctors use a (or multiple) standard ADHD Assessment Checklist. This ADHD checklist is part of the ADHD assessment tests that is completed with the doctor with observation of the patient also.
This is an ADHD Assessment Checklist that many doctors have their patients fill out. I remember filling out one like this. I thought it was going to take a week to fill out. You can print this page out and take this to the doctor with you.
Your doctor may have you fill out his/her own ADHD Assessment Checklist while you are in the office.
Remember, this is for things that are beyond what would be normal for the age of the patient.
For more information about the specifics on this, visit the ADHD symptoms and DSM-IV Criteria for ADHD page.
Please let me know if you experience any problems with this assessment so I can fix it. Contact me.
More than 20 checked items on the ADHD assessment tests indicates a strong tendency toward Attention Deficit Disorder or Attention Deficit Hyperactivity Disorder. The totals will automatically add up at the bottom.
If the total is more than 20, then you have a tendency towards ADHD. This ADHD assessment checklist does not confirm or diagnose ADHD. For a diagnosis, please see a doctor.
