34 y.o. w/ ADD..maybe depression.. or bipolar?

I'm 34 years old and I have a very hard time maintaining focus on things that I need to do.. And a lot of times things I am very interested in. It has really affected my life to the point where I am now separated from my wife and children for 3 months.

In addition I have been unemployed for a year and a half. These combined, I think, have driven me into depression. I have troubles discerning symptoms of add, adhd, depression, manic depression and bipolar disorder.. Is it possible I have more than one of the above?

I am so desensitized to what is wrong with me. All I know is that my life is falling apart around me. I was diagnosed with ADD when I was 16... At that time I was prescribed Cylert (now discontinued) and while I couldnt feel any direct effects people around definitely noticed a positive difference in my demeanor. I stopped when I was about 18. Then about 5 years later I was prescribed Straterra with did absolutely nothing for me.

I have no private insurance but I do have medicare. Would my primary refer me to a specialist?

Any advice is greatly appreciated. I just want to get my life together again.
